Where the nearest hospital is
The district headquarters hospital, DHQ Hospital, sits in Athmuqam, the administrative centre of Neelum district, roughly in the middle of the valley on the main Neelum Valley road. For anyone travelling from Keran, Sharda, Kel or further up, Athmuqam is the realistic first stop for anything beyond a minor injury.
For serious trauma, a major fracture or anything needing specialist treatment, the next step up is Muzaffarabad, about 73–74 km from Athmuqam by road, with fuller facilities than anywhere in the valley. That's a driving time of well over an hour from Athmuqam alone, more from the upper valley, so a bad injury far up the valley can mean several hours before proper trauma care.
Smaller villages have only basic government health facilities, adequate for minor cuts, fevers and common ailments but not for anything serious. Ask your hotel or guesthouse where the nearest one is when you check in.
What to do after a breakdown
- Pull off the road as far as you safely can. The Neelum road is narrow in places, and a stopped vehicle blocking a lane is a hazard, especially after dark.
- Put out a warning – hazard lights, or someone stationed a little way back on a blind bend.
- Flag down other drivers. Locals and other tourists generally stop to help, and someone with signal or local knowledge of the nearest mechanic is often more useful than a call for official help.
- Know roughly where you are. "Near Kundal Shahi" or "just past Keran" is far more useful to rescue services than "somewhere on the Neelum road," especially since signal drops make a follow-up call unreliable.

What to do after a landslide or blocked road
Landslides and rockfall are a real risk on this road, especially after heavy rain and in the monsoon months.
- Don't drive around a fresh slide or under a slope still shedding rock. Wait for it to settle before deciding whether to turn back or wait for clearance.
- Ask locals or other stopped drivers how long a similar closure has taken before. Some clear in an hour; others take a full day.
- Turn back to the nearest town with accommodation if it looks like a long closure, rather than waiting overnight in the vehicle.
- Report a serious blockage to 1122 or 15 if you have signal, particularly if other vehicles are affected or anyone is hurt.
What to do if someone is injured
For anything beyond a minor cut or scrape:
- Keep the person still, and avoid moving them more than necessary if there's any chance of a spinal or head injury.
- Call 1122 if you have signal. If not, flag down other traffic and ask someone to drive to the nearest reception, or to Athmuqam directly.
- Head for DHQ Hospital Athmuqam if the injury is serious but transportable, since it's the nearest facility equipped for more than basic treatment.
- For anything critical – a major fracture, a head injury with loss of consciousness, chest pain – aim for Muzaffarabad and get moving rather than waiting for an ambulance to reach you, since the drive time works both ways.
Carry a basic first aid kit in the car regardless of how short the trip looks. Cuts, sprains and minor camp-stove burns are the most common injuries, and a kit in the boot deals with all of them without needing a hospital at all.
Planning around the gaps
Emergency care in Neelum Valley gets thinner the further up you go, and the drive to serious trauma care is long. That's not a reason to avoid the valley, but it is a reason to travel with at least one other vehicle beyond Kel outside peak season, keep your tank topped up so a breakdown isn't self-inflicted, and tell someone your rough plan for the day before you set off.
